Required Paperwork for Participation in Athletics

2009-2010 School Year

• Completed physical form.  The physical date must be as of June 1st of  the new school year.
  
• Completed parent permission form.
  
• Mandatory liability meeting.  This meeting is required for parents of students who are participating in their first sport at Woodbury.
