 Change Forms

Address Change:

To change your address, everyone must complete the following forms:

Personnel Record Form

W-4 Form

 

If you are enrolled in the health, dental or life insurance, you must also complete the following form:

HealthTrust (LGC) Address Update/Correction Form

 

If you are enrolled in the New Hampshire Retirement System, you must also complete the following form:

NH Retirement System Change of Address/Name Form

 

If you have a New Hampshire Certification, you must also complete the following form:

NH Department of Education Name/Address Change Form

 

 

Name Change:

To change your name, everyone must complete the following forms:

If you are enrolled in the life insurance, you must also complete the following form:

HealthTrust (LGC) Life Insurance Application

 

If you are enrolled in the health and/or dental plan, you must also complete the following form:

HealthTrust (LGC) Medical and/or Dental Application Change Form

 

If you are enrolled in the New Hampshire Retirement System, you must also complete the following form:

NH Retirement System Change of Address/Name Form

 

If you have a New Hampshire Certification, you must also complete the following form:

NH Department of Education Name/Address Change Form

 

Beneficiary Change:

To change your beneficiary, you must complete the following forms:

NH Retirement System Beneficiary Form

HealthTrust (LGC) Life Insurance Application

 

If you are enrolled in the New Hampshire Retirement System, you must also complete the following form:

NH Retirement System Change of Address/Name Form
